Graphics card and fan burn out... psu problem?

Ok so i built my first pc and everything went well untill one day like 2 weeks ago it just shut off while i was playing a game. I turned it back on and after a second i saw some sparks and some smoke. After some testing on my second computer i figured out my video card blew out (not a big deal it was a nvidia 9800gt) I ordered a cheap card and some extra fans i added the graphics card it worked fine my pc is running fine then i installed a fan which also worked fine when first plugged it then i unplugged it to reposition it a little and when i plugged it back in it also sparked and stopped working..

Also when i look at my processor speed in control panel it says its at 3.21 when it should just be 3.20 Should i be worried about this? Could the psu be sending slightly more power then it should?

That psu is one of the best. It sounds like a short. Could also be power coming in from your plug atthe wall, fluctuations, but sounds like a short
