GPU fans go 100% and monitor goes black when starting a game. Problem then begins to happen right after POST

Hello, I recently built my first computer back in August of last year. As of this morning, when I tried to start the game For Honor, the computer monitor immediately black screened and GPU fans went 100% on me and I was unable to do anything besides hard reboot. After shutting it down right and getting past the motherboard logo screen, the computer monitor immediately black screened again and the GPU fans went to 100%.

I have reseated the GPU in its PCIE slot and removed all graphics drivers files for a clean install of the latest graphics drivers (I set my motherboard to Intel Graphics in order to do so). After doing this, the computer was able to make it past the motherboard logo screen for a single session of the game For Honor. When attempting to re-run the game an hour later, the black screening with 100% GPU fan speed started to reoccur in the same manner. I have had similar experiences with black screening with this GPU in the past while playing games (it started around 3 months after I bought it in August), but it never happened immediately after starting up a game. As of now, I am not sure what might be wrong with my computer. Any help would be appreciated.

Does the system work with the card removed from the system? If it does, contact the video card vendor for support or test the card in another system first.
