  1. GTX 1060 6GB cost a little more than $200 but is the best 1080 card out there right now. If not, the 1060 3GB and RX 480 are pretty good 1080 cards, if not quite guaranteed to run 60fps on all games.
  2. The GTX 1060 is a good bet, and get the 6 Gig for sure. When I crank up GTA V, it's using 3-4 gig of VRAM
  3. Question is a bit vague, the best gpu for $200 is the best, resolution will not affect that.

Currently the best two 60hz 1080p video cards are: AMD RX-480 and NVidia GTX-1060 6GB (Should have bore the name 1060Ti)
The next best two are the RX-470 and GTX-1060 3GB (Cores are different between the two 1060s in addition to RAM) These two will come close, but not quite the same levels of the best two.

RELEASE drivers had the 480 beating the 1060 6GB in DX12 and Vulkan, while the 1060 6GB beat it in DX11.... there are few reports, and even fewer still referred to, to reflect what newer drivers have improved or hurt in performance of the video cards in question. (That problem affects practically ALL cards on the market, past, preset, and, based off the past and present: future.)

Both the 1060 6GB and 480, are currently, the best, and up to the job. (GTX-1070 and 1080 are a bit overkill for 1080p 60Hz. We have to wait for Vega's release for anything better (new gen) than the RX-480.... otherwise you'll be looking at previous gen Fury series.)

Most of the time, the US version is updated monthly.... Last time it was updated was Jan 20, 2017 (as of when I post this)
http://www.tomshardware.com/reviews/best-gpus,4380.html.. and it puts the RX-480 as the best FHD (1080p)...and can be found for $200 or less (or more. The 1060 6GB's lowest cost is still around $250 USD.)
